Transcript Requests

Transcripts may be ordered in person, by mail, by fax and online (password required).

There is no fee for credit transcripts.

You may complete a transcript request form in the Admissions Office.

If you want a transcript mailed to an address other than a university or college,you must include a copy of a picture ID.

Mailed and faxed requests should include the following:

• your printed name
• current address
• a daytime phone number
• date of birth
• social security number or student ID #
• semester and course # and section (ie GOVT 2301-7001)
• signature & date
• Exact address to which the transcript is to be sent
