DCCCD cares about its students’ academic, emotional and physical success. The CARE Team was created to accept referrals regarding individuals in distress, struggling with basic needs, experiencing unexpected crises or whose behavior raises concerns about their well-being or that of others. The CARE Team also addresses behaviors or mental, emotional or psychological health conditions that may be disruptive, harmful or pose a direct threat or risk to the health and safety of the North Lake College community.

How Do You Make a Referral to the CARE Team?

Complete the CARE Team Online Referral Form.

A member of the CARE Team will review this information Monday-Friday within 24 business hours.

This form is not for emergency situations.

Who should be referred to the CARE Team?

If you become aware of one or more individuals displaying the following behaviors or concerns, please refer them to the CARE Team:

  • Changes in demeanor
  • Drastic changes to grooming or dress
  • Appears sad or depressed
  • Suicidal remarks or attempts
  • Emotional responses are unusual or exaggerated
  • Exhibits highly disturbing behaviors
  • Disrupts class
  • Submits disturbing written material
  • Making threats
  • Makes references to guns

Please also refer those individuals for whom you have general concerns. The CARE Team will connect with the individual and provide support.

The Counseling Center is also a consultation resource. They can assist you in approaching the distressed person or guide you to the CARE Team referral process.
